English or Hebrew alphabet OK.
Please enter the full date of death, including the year.
Enter as many people as you need. Please provide the full name, email address, and relation to the deceased of each person to receive a Yahrzeit notice.
Please select the appropriate payment amount.
It is understood and agreed that a "RESERVED" plaque will be attached to the Memorial Wall in our Sanctuary after payment is made in full. If an initial deposit is made with order, the balance will be required to be paid in full within one year from the date of deposit. If, at the termination of one year, the agreed price has not been paid to our office, any monies paid to date will be credited to the purchaser's synagogue account.
